AHA rejects Blue Cross claim that AI coding added $942Mn in costs

Key facts
  • The AHA disputes a Blue Cross estimate of $942 million in added coding costs.
  • BCBSA tied the 2023 to 2025 spending to AI documentation tools.
  • The AHA says BCBSA did not review medical records or identify AI claims.
  • Inpatient coding intensity added about 0.3 points to commercial spending growth, AHA estimates.

WASHINGTON, D.C., UNITED STATES — The fight over who pays for AI in hospital billing has reached the industry’s biggest trade groups, with hospitals rejecting an insurer analysis that blamed AI documentation tools for $942 million in extra spending. The American Hospital Association (AHA) says the real question is whether a patient’s medical record supports a diagnosis, not whether coding has become more complex.

Hospitals say sicker patients, not software, drive coding

The Blue Cross Blue Shield Association (BCBSA) estimated that coding-related changes added $942 million in spending for its plans between 2023 and 2025 and suggested that hospitals’ AI documentation tools were pushing costs up, according to an AHA blog post responding to the report. The AHA argues that BCBSA largely attributed rising patient complexity to coding behavior while overlooking well-documented changes in who ends up in hospital beds.

Americans are living longer with more chronic conditions, and many lower-acuity procedures have moved to outpatient departments, the AHA said. That leaves hospitals treating a sicker inpatient population, which it says explains why they report more complex conditions.

“Ultimately, the appropriate test for whether a patient has been correctly coded is not whether reimbursement increased; it is whether the patient’s medical record supports the diagnosis,” the AHA wrote. It added that BCBSA did not review underlying medical records or identify which claims involved AI tools rather than traditional coding.

A small slice of premium growth, the AHA says

The AHA also questioned the scale of the problem. Using BCBSA’s own March 2026 estimate, it calculated that inpatient coding intensity contributed only about 0.3 percentage points to commercial spending growth, compared with a 7% rise in employer family premiums in 2024.

The group then turned the argument on insurers, noting that Blue plans in Medicare Advantage benefit from fuller diagnosis capture through risk adjustment. It cited a federal audit of BlueCross and BlueShield of Alabama that found unsupported diagnoses in 247 of 271 sampled enrollee-years.

“Accurate diagnosis capture is not a coding problem; it is a patient care imperative,” the AHA wrote. Hospitals, it said, already rely on documentation rules, coding standards, clinical reviews and audits to keep reported diagnoses clinically supported.

For hospitals, the dispute means every AI-assisted code needs a clear trail back to the chart, because payers are now looking for patterns in coding intensity. Teams that outsource medical coding or wider revenue cycle management (RCM) work should ask partners how they audit AI suggestions and document clinical support before claims go out. Hospitals comparing vendors among the top U.S. healthcare outsourcing companies can make audit trails and coder review rates part of the scorecard.
