Canada’s Renfrew County launches remote care program for seniors

ONTARIO, CANADA — Renfrew County is stepping into the future of healthcare with the launch of a new remote care monitoring (RCM) program designed to help frail seniors safely transition from hospital to home, according to a report from Pembroke Today.
The initiative developed by Pembroke Regional Hospital (PRH) in partnership with St. Joseph’s Continuing Care (Cornwall) provides short-term, personalized virtual support for older adults recovering after hospitalization or emergency care.
Innovative remote care for at-risk seniors
Supported by the Ottawa Valley Ontario Health Team (OHT) and Ontario Health East, the 30- to 60-day RCM program delivers remote, technology-enabled care to patients aged 55 and above.
It specifically targets individuals at risk of hospital readmission, such as those with new or adjusted medications, chronic illnesses, or post-rehabilitation needs.
“This initiative is a game-changer for our patients,” said Sabine Mersmann, PRH President and CEO.
“It bridges the gap between hospital and home, ensuring that our most vulnerable seniors receive the care and guidance they need during a critical time in their recovery. By helping our seniors remain healthy at home and connecting them to community supports, we’re not only improving outcomes – we’re enhancing lives,” Mersmann added.
The program is free of charge for those referred by PRH Clinical Care Leads, Geriatric Emergency Medicine nurses, or the hospital’s Mobile Geriatric Day Hospital staff.
Patients receive a care package with a tablet, blood pressure cuff, oxygen monitor, and weight scale—tools that enable daily health tracking and communication with medical professionals.
Weekly nurse check-ins, medication management, and personalized care plans add an extra layer of coordinated support.
