New York rural healthcare shortages threaten elderly communities

NEW YORK, UNITED STATES — New York’s rural counties face a dire shortage of healthcare providers, with some regions lacking basic services like pediatricians and dentists, according to a new report by New York State Comptroller Thomas P. DiNapoli

The federal Medicaid cuts, which may have an exacerbating effect on the crisis, may close vulnerable hospitals and leave almost 30% of Americans living on the frontiers, who are largely elderly or low-paid, without a safety net.

Rural New York faces critical shortages in health providers

The healthcare system in rural New York is failing to cope with critical shortages in providers, as 16 counties have an average of only 4.0 primary care physicians per 10,000 residents, as compared to the 8.1 state ratio, the 8.4 United States ratio, and the Graduate Medical Education National Advisory Committee (GMENAC) guideline of 6.9. 

There are four counties, such as Hamilton, Herkimer, Schuyler, and Yates, without OB-GYN physicians, which means women have to leave their households to get to a care facility. Three counties lack pediatricians severely, while Hamilton County has no dentists, leaving residents without basic dental services.

Mental health access is equally dire, with rural areas having 6.9 practitioners per 10,000 people compared to 16.1 statewide. 

All 16 counties are designated Mental Health Professional Shortage Areas, with Sullivan County scoring 19 out of 25 for severity. DiNapoli warns these gaps contribute to higher rural suicide rates, compounded by isolation and economic strain.

Medicaid cuts threaten fragile rural hospitals

The reduction of Medicaid under Public Law 119-21 by the federal government may impact the healthcare system of the rural population, 27% of whom use the program. 

Six rural hospitals in New York are among the most dependent on Medicaid in the nation, with five already experiencing losses. DiNapoli predicts closures or downsizing without intervention, forcing patients to use emergency rooms for primary care—a costly and inefficient alternative.

The state’s Essential Plan is also at risk. A rollback may leave thousands uninsured, and it would disproportionately negatively affect rural populations, which have fewer other options available. 

Federal support, proposed as legislation for rural hospital funding at $10 billion per year until 2030, may not be sufficient to offset losses due to the unclear allocation of these funds.

Innovative solutions needed: Telemedicine and more

DiNapoli supports the idea of an expansion of telemedicine, but the availability of poor broadband in rural places does not support its potential. 

Mobile clinics and school-based health centers have the potential to fill the gaps, with few in operation now. Paratransit improvements are critical, as many counties lack reliable public transit for medical visits.

To attract providers, the report recommends loan forgiveness programs like Doctors Across New York and upskilling nurses through community college partnerships. 

Nevertheless, federal student loans for medical school were $200,000, lower than the average cost of $286,454, which falls within the scope of damaging recruitment initiatives. 

DiNapoli emphasized to North Country Public Radio, “Our rural counties may not be the population centers for the state, but still, every New Yorker should be entitled to access to quality health care.” He also notes that the state and federal governments should collaborate to prevent diseases in rural areas.
