Raintree buys Spike to add agentic AI voice to RCM

ARIZONA, UNITED STATES — Raintree Systems has acquired Spike Technologies, embedding the startup’s agentic artificial intelligence (AI) voice platform directly into Raintree’s electronic health records (EHR) and practice management system for rehabilitation and physical therapy providers.
Spike’s platform automates revenue cycle tasks including prior authorization, insurance eligibility verification, and claims follow-up, operating end to end within the EHR.
Agentic voice embedded natively in Raintree’s EHR
Physical therapy practices face an average claim denial rate of approximately 13%, with prior authorization consuming more than 10 minutes of staff time per patient visit.
Raintree, which serves more than 50 million patient visits annually across 8,500 clinics, is embedding Spike’s voice agents natively into its EHR to give them full revenue cycle management (RCM) context rather than deploying them as a third-party integration.
“Getting paid is the hardest, most manual part of running a therapy practice, and voice is where that burden is heaviest. We chose to take on the hardest piece first. We are not just optimizing operational efficiency for our clients, we are launching a seismic shift that will slash RCM expenses to an estimated 1%,” said Nick Hedges, Chief Executive Officer (CEO) at Raintree Systems.
Spike’s agents reason through complex denials end to end
Spike Technologies builds voice agents that complete prior authorization (PA) requests, insurance eligibility checks, and claims follow-up end to end inside the EHR, reasoning through denials without human intervention.
Integration inside the EHR gives Spike’s agents the full clinical and billing context needed to resolve complex PA cases at scale, addressing a gap that most AI prior authorization tools have not closed.

The acquisition reflects the maturation of agentic AI in healthcare revenue cycle, where automation is advancing from document support to active workflow execution inside clinical systems.
Embedding RCM automation at the EHR layer, with full patient and billing context, closes the gap between AI-assisted and fully autonomous payment workflows for therapy practices.
For the broader business process outsourcing (BPO) sector, agentic voice represents direct pressure on the outsourced revenue cycle operations that currently handle prior authorization, denials, and eligibility verification for United States health systems.
