NHS leans on U.S. hyperscalers for £442Mn in cloud

WASHINGTON, UNITED STATES — The National Health Service (NHS) in England spent £442Mn (approximately US$571Mn) on cloud services from the three major United States hyperscalers in its most recent financial year, while 77% of its 213 trusts and foundation trusts maintain active infrastructure connections to at least one of Microsoft Azure, Amazon Web Services, or Google Cloud, according to a report from Computer Weekly.

NHS cloud spend reaches £442Mn annually

A data analysis by Computer Weekly of the DNS records of all 213 trusts and foundation trusts in the NHS England provider directory reveals the concentration. Microsoft holds 1,889 of those records, against 229 for Amazon Web Services (AWS) and just 24 for Google Cloud.

That imbalance has prompted concern inside and outside Parliament, where scrutiny of NHS technology supplier relationships has intensified over the past 12 months.

“Microsoft had ripped off the NHS,” said Samantha Niblett, a Labour member of Parliament, during a House of Commons select committee session.

Of the 213 NHS trusts and foundation trusts analyzed, 132 (62%) route their email and identity management through Microsoft 365, effectively making Microsoft not one supplier among many, but a default platform for the National Health Service.

Parliament flags U.S. vendor lock-in risk

The NHS figures sit within a wider pattern of United Kingdom public sector hyperscaler dependence. Across central and local government, £9.9bn (approximately US$12.8Bn) of the £17.7Bn (approximately US$22.9Bn) spent with major technology suppliers in 2023/24 went to hyperscalers or their resellers, according to a broader Computer Weekly analysis of U.K. public sector contracts.

The Competition and Markets Authority (CMA) concluded that Amazon Web Services and Microsoft hold “significant unilateral market power” in the U.K. cloud market but opted for voluntary commitments over binding rules.

Under the U.S. Cloud Act, American authorities can demand access to patient data held by U.S.-based cloud operators on British soil.

The analysis comes as Microsoft announced a £700Mn-plus (approximately US$903Mn) five-year productivity tools deal with NHS England, covering Microsoft 365, Copilot Studio, and artificial intelligence (AI)-driven agent tooling for 505,000 clinical and administrative staff.

Health systems globally managing large-scale clinical data workloads are evaluating the balance between hyperscaler convenience and the data residency and sovereignty controls patient information requires. The NHS case illustrates how procurement decisions made for cost and scale can create strategic dependencies that are difficult to unwind.
