Sagility steers beyond healthcare outsourcing

BENGALURU, INDIA — Sagility, one of India’s largest listed healthcare services companies, is reorienting its delivery model away from labour-intensive outsourcing and toward artificial intelligence (AI)-enabled outcomes, positioning the company as an analytics and technology partner for United States payers and providers rather than a traditional headcount-led business process outsourcing (BPO) operator.

Outcome accountability replaces FTE delivery as Sagility’s commercial model

According to a profile published by BusinessWorld, Ramesh Gopalan, Managing Director and Group CEO of Sagility, outlined the company’s shift toward technology-led, outcome-accountable service delivery as the defining strategic direction for the business.

Sagility‘s repositioning centers on building AI-based use cases that automate clinical and administrative processes previously managed by human teams, including nurse-assist and appeals-assist tools, while expanding its analytics capabilities to support payer quality operations and provider revenue cycle functions.

The company added 17 new clients in the financial year 2026, including regional health plans and Blues plans, and generated revenue of approximately US$814 million for the full year, a 29.1% increase year-on-year.

The changing composition of Sagility’s business, toward higher-value analytics and AI-led work and away from volume-driven transactional processing, is the mechanism behind the company’s evolving margin profile.

Technology integration and accountability for outcomes define the next phase

Sagility’s investment in AI-led healthcare operations, including its June 2026 acquisition of CareSeed for Medicare Advantage quality analytics, signals that the company’s growth strategy runs through deepening technology integration rather than expanding headcount.

Sagility’s 46,860-employee base across India, the Philippines, and the United States gives the company the operational depth to combine offshore delivery with AI tools at a scale that smaller analytics-only competitors cannot replicate.

For healthcare buyers evaluating offshore BPO partners in the United States payer and provider market, Sagility’s repositioning reflects a broader shift underway across the sector: outcome-based accountability is becoming the differentiator that separates technology-capable partners from volume-based offshore operators.

The company’s US$814 million revenue base and direct access to payer and provider workflows put it among a small group of offshore operators large enough to credibly make the transition.
